Eligible first of month following one (1) month of employment.

Three plan options, from Blue Cross Blue Shield, with different deductible levels to choose from:

● Blue 200  (Highest employee contribution per pay)
● Blue 300
● Blue 400  (Lowest employee contribution per pay)

For each plan, you can choose from three (3) levels of coverage:

● Employee
● Employee + 1
● Family

Employee contributions will be determined based upon selection of the plan options and levels of coverage.

Ability to seek treatment within BC/BS network, out-of-network or with BSHSI facilities.
Medical Plan Opt-Out Program available.
